Certification
The undersigned requests technical support for a Lawfully Authorized Electronic Surveillance (LAES) under exigent circumstances, and certifies that:
- The undersigned is employed by the government entity named below and is authorized to declare an exigent circumstance;
- An emergency exists, pursuant to 50 U.S.C. § 1805(e), 18 U.S.C. § 2518(7), 18 U.S.C. § 3125, or a comparable state provision, that requires a Lawfully Authorized Electronic Surveillance to be conducted before a court order authorizing such surveillance can, with due diligence, be obtained; and
- There are grounds upon which a court order could validly be issued to authorize the requested Lawfully Authorized Electronic Surveillance.
The Service Provider named below and Subsentio may rely on this completed document for purposes of statutory immunity from liability related to the requested LAES.

Delivery
The Data Delivery Agency is the agency whose collection system will receive the delivery — for example NYPD, DEA HQ or Gladiator Forensics. An agency that does not already hold a current delivery VPN with Subsentio must either coordinate delivery through an agency that does, or request Subsentio's VPN form in order to establish one.
